Identify when to refer an employee to the EAP

 

A persistent pattern of several of these indicators, especially if

they are a change from the employee's usual level of functioning,

suggests that the supervisor  consider an informal or

formal referral to the EAP.  

 A.               Examples of job performance indicators

         1.  A  Pattern of absenteeism

         2. “On the job” absenteeism

      3.  Missed deadlines

         4.  Poor judgments

5.  Decreased efficiency

6.     Failure to follow instructions, directions, procedures

7.     Deterioration in quality of work

8.     Problems in concentration

9.     Impaired short term memory

10. High accident rate  

B.              Examples of physical indicators  

1.        Deterioration in personal appearance

2.        Excessive or rapid weight gain or loss

3.        Deterioration in motor coordination, speech, etc

4.        Significant changes in energy levels  

C.              Examples of psychological/interpersonal indicators  

      1.  Overreaction to criticism

                2.  Complaints from others about employee’s behavior

                        3.  Avoids co-workers, supervisor, colleagues

                     4.  Crying spells or unprovoked hostility

                    5.  Significant and rapid mood changes for no obvious
